Vouchers, Certificates, Coupon Codes and Terms and Conditions

From time to time, Galderma, Allergan, Skinbetter, Merz Aethetics, Endo Aesthetics, and Solta may offer certain Provider promotion certificates, rewards, rebates, vouchers, coupons, discount programs for Authorized Physician Partners ("APPs"), i.e. Ultra Smooth Skin employees and providers authorizing in house Treatment certificates, rewards, rebates, vouchers, coupons, discounts that are already included in the service/ product cost. These Provider promotion certificates, rewards, rebates, vouchers, coupons, discounts are applied and authorized for purposes of offering patients savings. The certificates, rewards, rebates, vouchers, coupons, discounts are available based on the treatment/service and can be combined if the promotion rules allow it.

Miscellaneous Vouchers

All Botox, Juvederm, Voluma, SkinMedica and Latisse vouchers (previously known as "Brilliant Distinctions vouchers"; now known as "Allē vouchers") can be submitted by the treating provider via the previous Brilliant Distinctions portal (now Allē) per authorization from Allergan. Allergan will reimburse the healthcare provider for the face value of the coupon provided that the provider and consumer have complied with these terms and conditions. Individual treatment certificate coupon codes are valid for either per product use or every three months. This offer is limited to voucher coupon codes per patient and cannot be combined with any other offer or promotion except the previous Brilliant Distinctions Rewards program (now known as Allē). Limit one treatment certificate coupon code per patient per treatment. Certain restrictions apply. Treatment certificate coupon codes have no cash value without simultaneous purchase of Botox, Juvederm, Voluma, SkinMedica, and Latisse are void if sold, transferred or exchanged. Offer valid only in the U.S., excluding territories, and void where prohibited, taxed or otherwise restricted by law.

Dysport/Galderma Specific Information

All Dysport treatment certificate coupon codes can be submitted by the treating provider via www.ASPIREhcp.com. This is per instruction, authorization and approval from Galderma. Galderma will reimburse the healthcare provider for the face value of the coupon provided that the provider and consumer have complied with these terms and conditions. Individual treatment certificate coupon codes are valid for either first time one-time use or every three months. This offer is limited to treatment certificate coupon codes per patient and cannot be combined with any other offer or promotion except the ASPIRE Galderma Rewards program. Limit one treatment certificate coupon code per patient per treatment. Treatment certificate coupon codes have no cash value without simultaneous purchase of Dysport or Restylane Silk or Restylane or Restylane Lyft or Sculptra treatment, and are void if sold, transferred or exchanged. Offer valid only in the U.S., excluding territories, and void where prohibited, taxed or otherwise restricted by law.

By participating in the certificates, rewards, rebates, vouchers, coupons, discount programs provided by Galderma, Allergan, Skinbetter, Merz Aethetics, Endo Aesthetics, and Solta, Ultra Smooth Skin and their agents, officers, providers, representatives, employees are not responsible for and shall not be liable for: time consuming entries, including current and past lost, hindered, misdirected, misaddressed, inaccurate, inappropriate, inaccurate training, misappropriated, incomplete, or incomprehensible registration or voucher entries; or any circumstances beyond our control, any injuries, claim of losses, problems or damages of any kind arising in connection with this benefit. Additionally, we, Ultra Smooth Skin and its agents, officers, providers, representatives, and employees are not liable for any damages of any sort (including financial loss or profit), punitive, or other damages arising from Ultra Smooth Skin in assisting in the management of these programs. You hereby waive all rights to claim punitive or incidental, or special damages, including lawyer fees and you further waive all rights to have damages compensated. By participating in any of these Programs, you agree to release, discharge, and hold harmless Ultra Smooth Skin and their agents, officers, providers, representatives, employees all liability from any injuries, losses, or damages of any kind arising from or in connection with the programs.
